FUTA tax wrong for just one employee

In all the 50 some employees, i have one that constantly has "extra" FUTA tax allocations, making the report double what they should be. I thought it would go away in the new year but now it has doubled again, yes, 4 extra entries now. I have verified the database, rebuilt, etc. I can't find any entries in this one person set up that is causing this. Started out of no where near the end of last year and continues. Again, I hoped that something would zero out and start out normal. Do I create a new employee for this to start over? Any ideas what is doing this?
Attached is a screen shot of it, in the company summary you can see only 2 of the 4! that are actually there.

Is this employee's profile using a different / unique job definition?
