Parts Randomly not included in build assembly

I have been using assembly builds for a few years and I have some parts that WILL NOT be included in the build assembly every time, some times they will then the next time they will not be included. they show on the build list and in the item list on the build list it show 0 being allocated.
I have 1 part that NEVER is included.
any ideas?
is the Cost being accounted for,
it is strange that it will allow the build but then not include it in the build.
what is the correct procedure for fixing the O/H count parts adjust out, bull to COG account??
